Requirements for Enrollment

• Old Student
  • Report Card (Form 138 or SF 9) and
  • Accomplished Registration Form
• New Student & Transferee
  • Report Card (Form 138 or SF 9)
  • Permanent Record (Form 137 or SF 10)
  • PSA Birth Certificate
    • Clear colored photocopy of the Birth Certificate issued by the Philippine Statistics Authority (PSA).
    • If the PSA copy is not legible, the applicant must submit, in addition to the clear photocopy of PSA issued Birth Certificate, a legible original copy of the Birth Certificate issued by the Local Civil Registrar (LCR).
  • Two (2) Recent I.D. Pictures (1X1)
  • Certificate of Good Moral Character
  • Interview with the Student Services Center
  • Entrance Examination
• Entrance Exam Schedule
  • Monday – Friday (8am – 5pm)
